1. MyBox insurance policies have an initial three-year term, and the premiums are paid on a monthly basis. No surcharges will be applied for splitting the payment. From the fourth year, the MyBox insurance policy will be renewed automatically every year. If the insurance policy is renewed, the renewal premium may be recalculated in accordance with the conditions of the policy. The MyBox Vida insurance policy has a maximum insurance limit of €2,000,000, and a minimum of €50,000. In the MyBox Decesos Seniors insurance, the full premium will be paid when the policy comes into force, and in the MyBox Decesos Seniors Flexible insurances, the full premium will be paid on a monthly basis for the first 36 months of the term of the policy. 2. Adeslas Salud y Bienestar is a service available to people over the age of 18 covered by the company policy with Adeslas. This service and digital health services are not covered in the areas of Asturias, Navarre and the Basque Country.

4. Cover only available for insured persons of up to 62 years of age, and the premium will be calculated taking into account the capital sum insured and the age of the insured person at that time. Illness cover for cancer, acute myocardial infarction, bypass and transplant; stroke; kidney failure; degenerative diseases (Alzheimer’s, Parkinson’s, multiple sclerosis and amyotrophic lateral sclerosis), and organ transplants are mutually exclusive, i.e. if one of these contingencies occurs, the others will be extinguished, and 50% of the capital provided for death or total permanent disability will be paid, with the remaining 50% of the capital being held exclusively for death and total permanent disability cover, accidental death and accidental total permanent disability, which will remain in force for that amount. Likewise, for cancer, acute myocardial infarction and other serious illnesses, a waiting period of 90 days will be applied. The cover for permanent absolute disability is for the case where the insured party suffers an irreversible physical situation, beyond their control, which prevents them doing any paid work as an employee or on a self-employed basis. Permanent absolute disability is not considered to be situations where the insured party is only prevented from carrying out their usual profession, but can do or perform other work, another profession or job (total permanent disability). Customers over 62 and up to 67 years old can only take out MyBox Vida with death coverage.
5. Double capital on account of an accident applies in cases where death or total permanent disability occurs no more than two years after the accident took place.
6. The additional second medical opinion service in the event of serious illness will be provided under the MyBox Vida insurance policy during the first three years after the policy is taken out, provided that the policy is in effect and the policyholder is up to date with payments. The additional service will be provided by the entity Teladoc-Health Care Management, S.A., or, if unavailable, another similar entity.
7.
